High-throughput sequencing defines donor and recipient HLA B-cell epitope frequencies for prospective matching in transplantation

چکیده
Abstract Compatibility for human leukocyte antigen (HLA) genes between transplant donors and recipients improves graft survival but prospective matching is rarely performed due to the vast heterogeneity of this gene complex. To reduce complexity, we have combined next-generation sequencing in silico mapping determine population frequencies probabilities 150 antibody-binding eplets across all 11 classical HLA 2000 ethnically heterogeneous renal patients donors. We show that are more common uniformly distributed than respective isoforms. Simulations targeted eplet shows a high degree overall compatibility, perfect identity at clinically important class II loci, can be obtained within patient waiting list approximately 250 subjects. Internal epitope-based allocation thus feasible most major programs, while regional or national sharing may required other solid organs.

High-resolution donor-recipient HLA matching contributes to the success of unrelated donor marrow transplantation.
The relative importance of various human leukocyte antigen (HLA) loci and the resolution level at which they are matched has not been fully defined for unrelated donor transplantation. To address this question, National Marrow Donor Program data from 3857 transplantations performed from 1988 to 2003 in the United States were analyzed. Patient-donor pairs were fully typed for HLA-A, -B, -C, -DRB...
متن کاملPersonalized Donor-Recipient Matching for Organ Transplantation
Organ transplants can improve the life expectancy and quality of life for the recipient but carries the risk of serious post-operative complications, such as septic shock and organ rejection. The probability of a successful transplant depends in a very subtle fashion on compatibility between the donor and the recipient – but current medical practice is short of domain knowledge regarding the co...
متن کاملHLA epitope based matching for transplantation.
As important risk factors for transplant rejection and failure, HLA antibodies are now recognized as being specific for epitopes which can be defined structurally with amino acid differences between HLA alleles. Donor-recipient compatibility should therefore be assessed at the epitope rather than the antigen level. HLAMatchmaker is a computer algorithm that considers each HLA antigen as a serie...
متن کاملBone marrow donor registries: discordance in HLA frequencies between donor and recipient populations.
We read with interest the article by Sonnenberg et a1 describing the relationship between registry size and the probability of finding completely and partially HLA-matched unrelated marrow donors.’ The effect of a significant difference in HLA profiles between donor and potential recipient populations was simulated by using two groups of different ethnic origin and was shown to drastically alte...
متن کاملReflections on HLA Epitope-Based Matching for Transplantation
HLA antibodies are primary causes of transplant rejection; they recognize epitopes that can be structurally defined by eplets. There are many reviews about HLA epitope-based matching in transplantation. This article describes some personal reflections about epitopes including a historical perspective of HLA typing at the antigen and allele levels, the repertoires of antibody-verified HLA epitop...
